Hepatobiliary oncology surgery for gallbladder cancer — Dr. Arefin's team

    Gallbladder Surgery

    Gallbladder Cancer Resection Dhaka

Gallbladder cancer is an aggressive hepatobiliary malignancy that requires radical surgical resection for curative treatment. Dr. Murshidul Arefin performs the full spectrum of gallbladder cancer surgery — from extended cholecystectomy to liver wedge resection and lymph-node clearance — at Popular Medical College Hospital, Dhanmondi.

    Who needs gallbladder cancer resection

    • Suspicious gallbladder mass or thickening on imaging
    • Incidentally found cancer after routine cholecystectomy (T1b or higher)
    • Gallbladder polyps >1 cm with malignant features
    • Porcelain gallbladder with mass
    • Selected metastatic disease for palliative bypass

    Surgical procedures

The extent of surgery depends on T-stage. T1a (mucosa only) is cured by simple cholecystectomy. T1b–T3 requires extended (radical) cholecystectomy — removal of the gallbladder en bloc with a 2 cm wedge of liver bed (segments IVb and V) and porta hepatis lymphadenectomy. Selected T3–T4 tumours invading adjacent liver may need formal right hepatectomy or extended right hepatectomy with bile-duct resection — overlapping with cholangiocarcinoma surgery.

    Multidisciplinary care

Every gallbladder cancer case at Dr. Arefin's practice is reviewed in a multidisciplinary tumour board with radiology, oncology, and pathology. Adjuvant chemotherapy (capecitabine) is recommended for T2 and above, or for node-positive disease. This integrated pathway gives the best long-term survival outcomes.

    Outcomes and follow-up

5-year survival is 80–90% for T1a, 50–70% for T1b–T2 with R0 resection, and 20–40% for T3 disease. Structured follow-up includes 3-monthly imaging and tumour-marker (CA 19-9) review for the first 2 years. Book a consultation for second opinion or surgical planning.
