Pancreatic cystogastrostomy and IPMN resection — Dr. Arefin in Dhaka

    Pancreatic Surgery

    Pancreatic Cyst Surgery Dhaka

Pancreatic cysts range from harmless inflammatory pseudocysts to premalignant mucinous lesions that can progress to cancer. Accurate classification is critical — overtreatment exposes patients to unnecessary surgery, while undertreatment risks missing curable cancer. Dr. Murshidul Arefin evaluates and operates on pancreatic cysts at Popular Medical College Hospital, Dhanmondi.

    Cyst types and malignant potential

    • Pseudocyst — inflammatory, post-pancreatitis; no cancer risk; treat only if symptomatic
    • Serous cystadenoma (SCA) — benign; observe unless very large or symptomatic
    • Mucinous cystic neoplasm (MCN) — premalignant (in women, body/tail); resect
    • Intraductal papillary mucinous neoplasm (IPMN) — premalignant; main-duct and mixed-type require resection, branch-duct surveilled or resected based on worrisome features
    • Solid pseudopapillary neoplasm (SPN) — low-grade malignant; resect

    Workup

MRI/MRCP is the gold-standard initial test. Endoscopic ultrasound with fine-needle aspiration samples cyst fluid for CEA, amylase, glucose, and cytology. High CEA (>192) suggests mucinous, low glucose strongly favours mucinous, and amylase identifies pseudocysts. International (Fukuoka/Kyoto) guidelines guide resect-vs-surveil decisions for IPMNs based on size, mural nodules, main-duct dilatation, and symptoms.

    Surgical procedures

    • Symptomatic pseudocyst — endoscopic or surgical cystogastrostomy (internal drainage into stomach), or distal pancreatectomy if disconnected
    • Body/tail mucinous cyst or IPMN — distal pancreatectomy (with or without splenic preservation)
    • Head IPMN or mucinous lesion — Whipple procedure
    • Diffuse main-duct IPMN — total pancreatectomy (rare)

    Surveillance for low-risk cysts

Branch-duct IPMNs without worrisome features and small SCAs can be safely surveilled with annual MRI. Dr. Arefin reviews surveillance imaging personally and operates only when malignant transformation features develop. Book a consultation with your imaging for a tailored plan.
